The Salem City School Division does not discriminate on the basis of race, color, national origin, sex, creed, marital status, age or disability in its programs, activities, or employment practices as required by the Title VI, Title VII, Title IX, Section 504, and ADA regulations. The School Board does not discriminate on the basis of sex in its education program or activity and we are required by Title IX not to discriminate in such a manner. The requirement not to discriminate extends to admission and employment and inquiries about the application of Title IX may be referred to the Title IX Coordinator, the Assistant Secretary for Civil Rights of the United States Department of Education, or both.

The Human Resources Office, Salem City Schools, at 510 South College Avenue, VA 24153, (540-389-0130), is responsible for coordinating the division's efforts to meet its obligations under Section 504, Title IX, the ADA, and their implementing regulations.

I authorize my listed references, current and past employers and educational institutions, and anyone else who has information about my work history, education qualification, or fitness to provide such information to the Salem City Schools. I release the Salem City Schools, and all persons providing this information to the school district, from any liability whatsoever for obtaining and providing that information, regardless of the results. affirm that all information set forth in this application is accurate, truthful and complete.

If employed by the Salem City School Board, I wili abide by all School Board Policies. I understand that the the terms of the contract or employment notice are subject to change should the information provided be inaccurate or not officially verifiable.

I hereby authorize and grant permission for the Salem City Schools Human Resources Office to conduct a personal record check from federal, state, county, and/or local law enforcement agencies and the Department of Social Services and waive the right to hold liable individuals listed as references and current or former employers from any liability for information given in response to a request for an employment reference.

In the event that I am employed by Salem City Schools and in the further event that I have provided false or misleading information in this application or in subsequent employment interviews, I understand that my employment may be terminated at any time after the discovery of the false or misleading information. I understand that this application will be considered active for one year from date of submission.
